It is preferable not to have distant spread. which can easily be assessed by the PET scan or Total body MRI. 3. It is not suitable for genetic related breast cancer and a type of breast cancer (Lobular) – which can be detected by a simple blood test showing BRCA 1 and BRCA 2 gene expression.reast cancer is the most to removal of the entire breast. i.e. common cancer in Tamilnadu Women and its incidence shows a steep increase in the recent past. This is due to a change in the environment, social and dietary factors of our women towards that of western countries. The Tamilnadu women behave differently from western women from various aspects of breast cancer – mainly they do better in the outcome of the treatment. Hence it is important for our women to know about their disease, its behavior, early signs and symptoms, investigations, and modalities of treatment available including the latest one. All of us are aware that the breast cancer managment is a multimodal one among which the surgery plays the major role. The days of radical surgery are gone. Now it is the period of Breast Conservative Surgery and procedures like onco plasty, etc., In spite of all the evolutions people think that the Breast Conservative Surgery is not a reality, and it is only a Myth. They also feel that it cannot be practiced in our set up. This article is mainly to clear this doubt. Breast conservative surgery is a form of cancer surgery where the cancer tissue is removed with a margin of normal tissue all around along with removal of the affected lymph nodes without removing the entire breast. Oncoplasty procedure means filling the defect of the excised area with local flaps or altering the shape of the opposite breast accordingly. Both the above procedures have got the same oncological outcome equal 4. It is preferable not to have open biopsy done elsewhere The techniques of fine needle biopsy are being taught to all Medical Graduates and Postgraduates. 5. To practice conservative breast surgery, facilities for good radiotherapy, onco pathologist to study the tumour margins and medical oncology facilities must be available. Then why there is Resistance to Breast Conservative Surgery: 1. The patient feels that if there is residual breast, she may develop cancer in that it is not so because both breast conservative surgery and breast removal are equvalent in terms of survival and cure (proved by many international trials). 2. Patient relatives consult somebody who had undergone breast removal (mastectomy) before many years and living long so far without any problem and they insist for mastectomy – this is because in yester years, mastectomy was the only treatment avilable for the breast cancer. 3. Even some of the practising family physicians, advise their patients to undergo only mastectomy and not conservative breast surgery – that is why we are conducting lot of CME Programmes for the practioners and younger surgeons. 4. Proper expertise for breast conservative surgery and oncoplasty procedures are not available everywhere i.e. why mastecotmy being performed a lot. International and national trials conducted all over regarding the management of breast cancer clearly show that Breast Conservative Surgery Onco Plasty procedures give equal results as that of mastectomies, and they are no way inferior treatment. Hence it is only the patient’s and practitioner’s mindset to practice conservative breast surgery for benefitting the thousands of breast cancer patients not to lose their femininity.
